When you validate a schema from Visual Studio, it calls some HIPAA
utilities 
in the background.  Errors in these utilities don't often bubble up to 
Visual Studio.

'Validate Schema' action in Visual Studio calls XSD2EDI.exe to update
HIPAA 
tables, then COMPEIF.exe to generate a compiled file of the schema just 
registered.

You mentioned running XSD2EDI.  Now try running COMPEIF.exe manually:
1.  open a command prompt
2.  navigate to %esp_srv_hipaa%
3.  execute COMPEIF.exe

Get any errors ?
Make sure the HIPAA service account is a member of the following groups:
+ Administrators group (local to the BizTalk server)
+ BizTalk Application Users
+ BizTalk Server Administrators
+ HIPAA EDI Subsytem Users
+ SSO Affiliate Administrators
